It is likely that he was advised of the promotion, but it was not listed in the London Gazette in order to make it official prior to his death. It is not uncommon, that being the case his rank will remain that of Lieutenant.
His MIC shows his rank as Lt., ODGW has Lt. (TP).

The rank by which he is listed by CWGC will be as detailed to them by the Army. It is not unusual for the family ( and consequently war memorials) to promote the higher rank. I am aware of similar cases.

I fully agree Jim, I have seen a lot of such cases myself. Also temporary, local and acting ranks are not usually used by CWGC although I have seen exceptions too. I was just passing on a mail from a correspondant verbatim.
